Kizuna Clinic (hereinafter referred to as “the Clinic”) establishes the following Privacy Policy (hereinafter referred to as “this Policy”) regarding the handling of users’ personal information in the services provided on this website (hereinafter referred to as “the Services”).

Article 1 (Personal Information)

“Personal Information” refers to “personal information” as defined in the Act on the Protection of Personal Information, meaning information about a living individual that can identify a specific individual by name, date of birth, address, telephone number, contact details, or other descriptions contained therein, as well as data related to appearance, fingerprints, voiceprints, and information such as insurer numbers on health insurance cards that can identify a specific individual by itself (personal identification information).

Article 2 (Methods of Collecting Personal Information)

The Clinic may ask users for personal information such as name, date of birth, address, telephone number, email address, bank account number, credit card number, driver’s license number, etc., when users register for use. In addition, the Clinic may collect transaction records and payment-related information that include users’ personal information from partners of the Clinic (including information providers, advertisers, and advertising distributors; hereinafter referred to as “Partners”) and others.

Article 3 (Purpose of Collecting and Using Personal Information)

The purposes for which the Clinic collects and uses personal information are as follows:

  1. To provide and operate the Clinic’s services
  2. To respond to inquiries from users (including identity verification)
  3. To send emails informing users of new features, updates, campaigns of the services they are using, and other services provided by the Clinic
  4. To contact users as necessary regarding maintenance or important notices
  5. To identify users who violate the Terms of Use or attempt to use the services for fraudulent or improper purposes, and to refuse their use
  6. To allow users to view, change, or delete their registered information and view their usage status
  7. To bill users for usage fees for paid services
  8. For purposes incidental to the above purposes of use

Article 4 (Change of Purpose of Use)

  1. The Clinic shall change the purpose of use of personal information only when the changed purpose is reasonably recognized as being related to the original purpose.
  2. When the purpose of use is changed, the Clinic shall notify users of the changed purpose or publicly announce it on this website by the method prescribed by the Clinic.

Article 5 (Provision of Personal Information to Third Parties)

  1. The Clinic will not provide personal information to third parties without prior consent of the user, except in the following cases or as permitted by the Act on the Protection of Personal Information or other laws and regulations:
    1. When it is necessary to protect a person’s life, body, or property and it is difficult to obtain the individual’s consent
    2. When it is particularly necessary for improving public health or promoting the sound growth of children and it is difficult to obtain the individual’s consent
    3. When it is necessary to cooperate with a national government organization, local government, or a person entrusted by them to perform duties prescribed by law, and obtaining the individual’s consent may impede the performance of such duties
    4. When the following matters have been announced or disclosed in advance and the Clinic has submitted a notification to the Personal Information Protection Commission:
      1. That the purpose of use includes provision to third parties
      2. The items of data to be provided to third parties
      3. The means or methods of provision to third parties
      4. That provision of personal information to third parties will be stopped upon request by the individual
      5. The method for accepting such requests
  2. Notwithstanding the preceding paragraph, the following cases shall not be regarded as provision to a third party:
    1. When the Clinic entrusts all or part of the handling of personal information within the scope necessary to achieve the purpose of use
    2. When personal information is provided due to succession of business as a result of a merger or other reasons
    3. When personal information is jointly used with a specific party, provided that the fact of joint use, the items of personal information jointly used, the scope of parties jointly using it, the purpose of use by those parties, and the name or designation of the person responsible for managing the personal information are notified to the individual in advance or made easily accessible to the individual

Article 6 (Disclosure of Personal Information)

  1. When the Clinic is requested by an individual to disclose their personal information, the Clinic shall disclose it to the individual without delay. However, if disclosure falls under any of the following cases, the Clinic may decide not to disclose all or part of the information, and shall notify the individual without delay if such a decision is made. A fee of 1,000 yen per request will be charged for disclosure.
    1. When disclosure may harm the life, body, property, or other rights or interests of the individual or a third party
    2. When disclosure may significantly hinder the proper execution of the Clinic’s business
    3. When disclosure would otherwise violate laws or regulations
  2. Notwithstanding the preceding paragraph, information other than personal information, such as history information and characteristic information, will not be disclosed in principle.

Article 7 (Correction and Deletion of Personal Information)

  1. If a user’s personal information held by the Clinic is incorrect, the user may request correction, addition, or deletion of the personal information (hereinafter referred to as “Corrections, etc.”) in accordance with the procedures prescribed by the Clinic.
  2. If the Clinic determines that it is necessary to respond to the request described in the preceding paragraph, it shall promptly make the Corrections, etc. to the relevant personal information.
  3. When the Clinic makes Corrections, etc. based on the preceding paragraph, or decides not to make such Corrections, etc., it shall promptly notify the user.

Article 8 (Suspension of Use of Personal Information, etc.)

  1. If an individual requests suspension or deletion of the use of personal information (hereinafter referred to as “Suspension of Use, etc.”) on the grounds that the personal information is being handled beyond the scope of the purpose of use or was obtained by improper means, the Clinic shall promptly conduct the necessary investigation.
  2. Based on the results of the investigation, if the Clinic determines that it is necessary to comply with the request, it shall promptly carry out the Suspension of Use, etc. of the relevant personal information.
  3. When the Clinic carries out the Suspension of Use, etc. based on the preceding paragraph, or decides not to do so, it shall promptly notify the user.
  4. Notwithstanding the preceding two paragraphs, if the Suspension of Use, etc. would involve excessive costs or is otherwise difficult to implement, and if alternative measures necessary to protect the user’s rights and interests can be taken, such alternative measures shall be implemented.

Article 9 (Changes to the Privacy Policy)

  1. The contents of this Policy may be changed without notifying users, except as otherwise provided by laws and regulations or this Policy.
  2. Unless otherwise specified by the Clinic, the revised Privacy Policy shall take effect from the time it is posted on this website.
